Current Students & Recent Graduates
If you are a current student or graduated during the most recent school year,
you can obtain a copy of your transcript through the
Neshaminy High School Counseling Center.

Former Students & Graduates
Graduates who did not graduate during the most recent school year
should complete the Neshaminy School District Transcript Request Form.
Colleges, Employers & Other Organizations
Colleges, prospective employers, screening contractors, and other organizations
requesting education verification or student records must submit all required
information listed below.
Incomplete submissions cannot be processed by the District.
The request must be legible.

Student Name:
Full legal name of the student while enrolled in the District.

Date of Birth:
Student's date of birth in standard MM/DD/YYYY format.

Graduation Year:
Year of graduation, if known.

Recipient Details:
Full name of the person or agency receiving the records,
including the complete mailing address.

Signed Release:
A signed, written authorization from the former student
granting permission to release their records.
Privacy Notice
Do not include Social Security Numbers on any request forms
or supporting documentation. They are not required for verification.
